Neighborhood Health

Hartford HealthCare Neighborhood Health provides Street Medicine outreach for people experiencing homelessness. Our team visits encampments, bus stations, parks and other community locations to meet people where they are with care, supplies and connection to resources.

The team travels in a refurbished ambulance outfitted with medical equipment and supplies. Not everyone is ready for medical care right away, so outreach also helps build trust, meet basic needs and create a path to future care.

Since launching in August 2024, the Street Medicine team has provided more than 1,140 clinical interventions, including wound care, chronic disease support, mental health, addiction services and urgent care needs.

  • Mental health and addiction support
  • Program information and clinic referrals
  • Hygiene items
  • Food, snacks and clothing
  • Connection to housing support, harm reduction agencies, social services and other community partners

Where we provide Street Medicine

The Street Medicine team currently provides care in Hartford, with plans to expand to other areas in need over time.

Community partnerships

Street Medicine works with harm reduction agencies, housing support organizations, social service agencies and other street medicine providers to help address the many needs people experiencing homelessness may face.
