Hartford Hospital Earns Exemplar Status for Older Adult Care

Hartford Hospital earned Exemplar status for its work implementing high-quality interventions and initiatives to provide advanced, complete care for older adults.

The recognition is the highest level offered by the Nurses Improving Care for Healthsystem Elders (NICHE) program. Hartford Hospital has been part of NICHE for 20 years and enjoyed Exemplar status for 10.

“We are the only NICHE Exemplar site in the state of Connecticut, making us the preferred place of care for hospitalized older adults,” says Anna-Rae Montano, PhD, RN, coordinator of inpatient geriatric services at Hartford Hospital.

The hospital received the NICHE-ABIM Foundation Choosing Wisely® Trailblazer Award in 2020 for research using the Therapeutic HUB, and in 2021 for the SAFER Mobilization initiative. The Department of Geriatrics is dedicated to providing education, resources and mentorship for staff to further improve the care for older adults, Montano says.

As an Exemplar NICHE member, the Hartford Hospital team will continue to provide evidence-based, interdisciplinary approaches that promote better outcomes, positive experiences and improved care for older adults.

“The leadership and staff have demonstrated a tremendous ability to meet the needs of older adults,” says Mattia J. Gilmartin PhD, RN, executive director of NICHE. “Their capacity to bring together geriatric nursing resources and support to enhance care marks them as a leader in the field and signals their resolve to provide high-quality, person-centered care for older adults.”
